About Us

Identity theft reports

The Office of Student Administrative Services Financial Aid Compliance is committed to protecting your personal information, and we take reports of identity theft very seriously. If you believe someone has fraudulently used your identity to create a student account at University of Phoenix, submitting a claim by completing and returning an Affidavit to us is the first step.

Once we receive your Affidavit, we will conduct an investigation, which may include analysis of University records, IT data, interviews, etc. If we confirm that identity theft has taken place, we’ll correct our records accordingly and take additional available steps, including updating your credit report where necessary. We will also provide you with instructions about steps that you should consider taking.

If you recently received a 1098-T tax form and you believe you may be the victim of identity theft, please see below for instructions on how to formally report identity theft with the University. Provided we substantiate identity theft during our investigation, we will work with the IRS to revise and refile a corrected 1098-T, as needed. Importantly, please also note that the IRS issues reporting guidance for taxpayers who have been victimized by identity theft: opens in new window. Please see “Steps for victims of identity theft” and take appropriate action. In the meantime, the 1098-T can be ignored and need not be filed with your tax return.
